1. Fill out the order form below and either email me the form itself or the information on the form at beautifulfaith@gmail.com (If you prefer fax, pleas fax at 408 - 376 - 3674)

2. Email confirmation should arrive within 1 business day with your order along with my address to send payment

3. Once you recieve confirmation from me, send payment (I will send either an email or soompi pm to notify you that I have recieved payment)

4. I will send out your shipment

5. Once you recieve it, please whitelist me, if you are a soompi member; if not, enjoy your beautiful silkscreened masterpiece!
